PATIENT-CENTERED MEDICAL HOME (PCMH): INTEGRATING MEDICATION MANAGEMENT TO OPTIMIZE ADHERENCE OUTCOMES IN A PCMH WEB ACTIVITY

Target Audience

Physicians, pharmacists, registered nurses, case managers, and other care team members involved with patients requiring treatment for chronic illness.

Purpose Statement

A new strategy that has emerged to focus on care delivery, payment reform, and the use of health information technology that is gaining momentum is the Patient-Centered Medical Home (PCMH). The Patient-Centered Primary Care Foundation (PCPCF), a national coalition formed to improve the future of the American medical system, has recently identified the need to integrate medication management within the PCMH to advance this model as a comprehensive care solution. Health care professionals practicing within a PCMH setting need medication management instruction and tools to assist with identifying the right patients, coordinating the right care, and monitoring the right follow up, in order to ensure successful short and long term medication outcomes for chronic illness. This activity will examine and recommend innovative PCMH medication management strategies and applications including communication, collaboration, health information technology, and follow-up requirements with the ultimate goal of reducing the burden of disease caused by chronic illnesses.

Method of Participation

There are no fees for participating and receiving CME/CE credit for this activity. Upon successful completion of the post test (score of 70% or better), an electronic statement of credit will be made available immediately.
